Community Health Assessment online survey being conducted by Cayuga County Health Department and Auburn Community Hospital

Cayuga County Health Department and Auburn Community Hospital need your help! This survey is part of the Community Health Assessment process being conducted by Cayuga County Health Department and Auburn Community Hospital. Hearing directly from the people who live here will help us design programs and provide services that will improve the health of the community. This survey should take you about 15 minutes to complete.

COVID has been the focus over the last two years and has caused significant health issues in our community. However, the nature of this survey is to address not only COVID and its impacts but other issues that impact the health of our community members. We want to hear about your COVID experiences and have dedicated a whole section of the survey to COVID-related questions, but we also want to know what else is important to improve the access and delivery of health-related services in our community.
